Enrollment Requirements for ETK, TK & Kindergarten
 
Legal Proof of Age Birth Certificate, Baptismal Certificate, Passport, Statement by the County Registrar or Recorder or Notarized Affidavit from the parent or guardian.  HOSPITAL CERTIFICATES OF BIRTH ARE NOT ACCEPTABLE.
Proof of Residency Current Gas, Electric, or Water Bill and/or Notarized landlord verification rental agreement
Transfer Coming from a non LAUSD School
Required Immunizations Polio           4 Doses DTP              5 Doses MMR                  2 Doses Hepatitis B  3 Doses Varicella Vaccine       2 Doses
